Employment Source Terms & Conditions

The following terms and conditions form the basis for the Employment Source supplying contract temporary help to client
companies. The signature of the client or his agent below constitutes full acceptance of the following:
1. It is agreed that the insurance furnished by Employment Source does not cover the
operation of any motor vehicles licensed for highway use by any Employment Source
employee, nor does it cove physical loss or damage of machinery, equipment, or
materials while in the care, custody, or control of an Employment Source employee.
A certificate of automobile liability insurance is required form those clients who allow
Employment Source employees to drive, and it is agreed that the client accepts full
responsibility for bodily injury, property damages, fire, theft, collision, or public liability
claims arising out of the operation of a licensed motor vehicle for the client.
2. The Client agrees to indemnify the Employment Source and Employment Source
employees for injuries incurred by Client's employees in the course of their employment
resulting form work performed by Employment Source employees in a reasonable,
prudent manor and/or as instructed by client, and from losses resulting from willful
misconduct, intentional or negligent acts except for losses covered by Employment
Sources Worker's Compensation Insurance.
3. The Client also agrees not to entrust Employment Source employees with unattended
premises or any part thereof, or with the care, custody, or control of cash, negotiable, or
other valuables without prior permission of Employment Source.
4. Client agrees to furnish Employment Source employees with a suitable place to work
which shall comply with all laws and ordinances related to occupational health and
safety.
5. It is agreed that the client will notify the Employment Source of any changes in the duties
of the employees from those originally described to Employment Source. Client agrees
to negotiate a "buy out" should they choose to hire a temporary or temp-to-hire
employee and place them on their payroll.
6. Client agrees that they will pay Employment Source billing on receipt of invoices, and
that a service charge per month (18% per annum) will be paid on charges unpaid 30
days after invoice date as well as all collection fees,
attorney fees, and other collection expenses incurred in the collection of charges on this
client's account(s).
